Social Security Number (SSN) and Lawful Presence Documentation Request
MoDOT/MSHP Medical and Life Insurance Plan members may receive a letter the week of June 1, 2009, requesting SSN and lawful presence documentation for subscribers, spouses and/or dependent(s) currently enrolled in the Plan.
The letter WILL NOT be mailed to all plan members. Plan members in the Active Subscriber Only or Retiree-Subscriber Only Plan will not receive this correspondence, as social security information and lawful presence documentation is already on file for subscribers as part of the MoDOT/MSHP hiring and retirement process. All other Plan members will receive one of two versions of the letter--based on whether the subscriber is required to submit proof of lawful presence.

Coventry has been selected to provide both the administrative services as well as the network of providers to our plan members starting January 1, 2009. MoDOT Employee Benefits staff is working hard with Coventry to make this transition as flawless as possible. The website allows you to check providers only, but additional information and services will be added for the January 1, 2009 transition.

New Employee Enrollment

If you are a new Employee, you must enroll in SEBES within 31 days of employment with MoDOT/MSHP. You can make changes to your coverage during the first 31 days of your hire date. Coverage is effective the first of the month following receipt of the change. After this time, you can only change plans if you have a change of address during the next Open Enrollment period.
